1. How do I apply for Funding?

Complete an Application for Funding. With your completed application, also submit:

  • If it is your first application with CITT, include a current evaluation from the licensed therapist your child will be seeing. For re-applications, submit a current progress report. The therapist must be a CITT-contracted provider.
  • A photograph of your child.
  • The first page of your federal tax return for income verification purposes.

Please allow one month to process your application. You will be notified by mail of our decision.

2. Who is Eligible for Funding?

Children ages 0-12 in Maricopa County receiving physical, occupational and/or speech and language therapy are eligible for funding.

Families with an adjusted gross income of $100,000.00 or less are eligible for funding.

Once your application package is received, it is reviewed by board certified therapists. If criteria are met, partial funding of the child’s therapy will be provided for a specified period of time.

3. Can I use any Provider?

You can use any provider that is currently contracted with CITT. If your provider is not contracted with CITT, but would like to be, they can download the Provider Agreement and Provider Application from our website, or call us at 1-866-540-8922, and we will mail the paperwork to them.

You may switch providers during the six-month funding period as long as the new provider is contracted with CITT.

4. How do I become a CITT Provider?

Complete a Provider Agreement for your agency, as well as a Provider Application for each therapist that will provide therapy services to CITT-funded children. You will be notified by mail of our decision.

5. How can I help CITT?

There are many ways you can help CITT achieve the goal of supporting children and families with special needs.

6. How much does CITT fund?

CITT funds 50% of a parent’s out of pocket expense, not to exceed $45.00 for a one hour session, or $30.00 for a 30-minute session.

A maximum of 2 visits per week per discipline are funded. If a therapist highly recommends that a child be seen more than twice a week per discipline, a request may be made to the Board of Directors for special approval. Requests do not guarantee funding.

Funding is provided in six-month increments. After six months, re-application must be made at least 30 days prior to expiration of funding in order to receive continued funding.

7. How do I become a Corporate Sponsor?

There are many ways an organization can contribute financially to CITT. For information on becoming a Corporate sponsor of CITT, click here or email kathy@cittkids.org with your interest and availability.

In order to reduce expenses and to ensure that over 96% of our funds go directly to serving children with special needs, CITT does not have a traditional "bricks and motor" office.  The phone system we utilize is a message system.  A member of our board checks this message system on a daily basis.   We apologize for any inconvenience this may cause.

9. What is the DMTE Program?

In 2003, CITT established a Durable Medical and Therapy Equipment (DMTE) Recycling/Loan Closet to provide equipment in support of therapy. The types of equipment available is generally not covered by insurance or provided in other funded programs. Through this program, a family is able to borrow the equipment for the necessary length of therapy. To view a list of available equipment and to apply, click here .

10. What is the Megan McCord Fund?

100% of all funds donated to the Megan McCord Fund are specifically designated towards the cost of therapy for a child with special needs.

This fund was created in honor of Megan McCord, a delightful ray of sweetness and light who lost her life in 2003 at the age of twelve. Her parents requested that all donations in Megan’s memory be given to CITT, establishing the Megan McCord Fund.
